The Effect of Alkaline Urine over pH 8.0 on the Test for Proteinuria


Abstract
Screening test for proteinuria by using reagent strip(RS) has been a important indicator of renal disease. The principle of RS test to detect proteinuria is based on pH change in the presence of protein, mostly albumin. Akaline urine has been reported to affect RS test for proteinuria compared with sulfosalicylic acid(SSA) test. 120 alkaline urine specimens were examined by using RS and SSA tests, simultaneously. As a negative control, 120 urine specimens with normal pH also were evaluated. For RS test, Clinitek l00(Bayer, USA) automatic machine was used. Of 120 alkaline urine specimens, 43(35.8%) were positive in RS test, while 34(28.3%) were positive in SSA test. Of 120 urine specimens with normal pH, 20(16.6%) showed positive reaction in SSA test. In conclusion, alkaline urine causes false positive reaction for proteinuria in RS test. Therefore, in the cases of alkaline urine specimens, SSA test is recommended for a more exact detection of proteinuria.
